Are you sure you have flat worms 0_o. It sounds like you have planaria and some harmless nematodes feeding off excess food/nutrients. Flatworms are salt water creatures. It's practically impossible to identify Macrobrachium spp., especially juveniles, from written descriptions. In many cases, only detailed physical examination of the actual specimen will suffice. Can you provide an image?

It definitely belongs to some species of Macrobrachium. I would separate it from your fish, as it probably consumed your neons.
